For Preschool:
A. Photocopy of PSA Birth Certificate
B. Completely Filled-out Forms (A1, P1, E1, H1). Please see the attachments above.


For Grade School and High School:
A. Photocopy of PSA Birth Certificate
B. Photocopy of Latest Report Card (Form 138) from the Current School
C. Photocopy of Certificate of Good Moral Character
D. Completely Filled-out Forms (A1, P1/P2/P3, E1, H1). Please see the attachments above. 

 

REQUIREMENTS FOR ENROLLMENT

1. Original Copy of Report Card (Form 138)
2. Original Copy of Certification of Good Moral Character by the Principal
3. Photocopy of Baptismal Certificate / Certification of Affiliation in a Religious Group
4. 5 pcs. (1×1”) colored ID photo with white background
5. 1 Recent Family Photo
6. Medical Certificate of Good Health
7. ESC Certificate (If applicant is an ESC grantee from a private ESC school)


OTHER REQUIREMENTS FOR FOREIGN STUDENTS/NON-FILIPINO CITIZENS
  1)  Photocopy of the Alien Certificate of Registration (A.C.R)
  2)  Study permit issued by the Bureau of Immigration (B.I.)
  3)  Photocopy of student’s and parents’ passport number, date of issue, place 
       of issue and type of visa (bring original passports for verification)  
  4)  Letter request from the duly appointed guardian/parents if in the country (BOI Requirements)
  5)  Affidavit of support and the proof thereof/Guaranty of return to country of origin
  6)  Affidavit executed by the parents appointing said guardian to take custody and care 
       over the minor-applicant (if executed outside the Philippines, the same must be duly authenticated to    
       be admitted as foreign document/s; if executed in the Philippines, parents’ passports must be   
       presented)
  7)  Photocopy of the guardian’s passport reflecting authorized stay, if guardian is an alien and not 
       a mere 9(a) visa holder
8)  Authenticated scholastic records (with English translation of the student’s name, 
       subjects, grades and grade/year level) by the Office of the Philippine Embassy / 
       Office of the Consular Official in the applicant’s country of origin, or place of residence, 
       which has jurisdiction over the issuing school
  9)  Students with dual citizenship must submit the Identification Certificate as a Filipino Citizen 
       issued by the B.I.                  
 

The decision of the Admissions Committee is final. Student transferees denied admission in previous years due to failed entrance test results, academic or behavior reasons, need not reapply.
